Community Care Northumberland’s ‘Caring Fore Our Community’ Golf Tournament Raises $18,100 for Essential Programs in Northumberland County

Community Care Northumberland (CCN) is thrilled to announce the success of its annual 'Caring Fore Our Community' Golf Tournament, held on June 20, 2024, at Shelter Valley Pines Golf Club in Grafton. The event raised an impressive $18,100, ensuring the continued affordability and accessibility of CCN's programs and services for clients throughout Northumberland County.

CCN is dedicated to providing essential services such as nutrition, transportation, wellness programs, in-home assistance, and hospice care, supporting seniors, adults with disabilities, and individuals recovering from illness or injury to maintain independence in their homes and communities.

Location: Shelter Valley Pines Golf Club

Registration Fee:  $150 per golfer, includes 18 holes, lunch, cart

Meal Choice: Steak, Chicken, Vegetarian Burger, Vegetarian Wrap

*Please note we offer a charitable tax receipt for $40 per registration that will be processed after you register. Due to CRA guidelines, the receipt will be issued in the name of the person paying for the registration. If you have any questions, please contact our office at 365-400-1592. 

Please arrive promptly at the course on the day of the event to ensure a smooth check-in process. You'll have the opportunity to meet fellow participants, enjoy some light refreshments and purchase your $20 activity package for special contests on the course.

Community Care Easter Cookies Fundraiser Provides 800 meals for Meals on Wheels!

FOR IMMEDIATE RELEASE: Northumberland County, ON – April 22, 2024 - Community Care Northumberland (CCN) is thrilled to announce the success of its Easter Cookies fundraiser, breaking records and making a significant impact on the community. Through the collaborative efforts of CCN, Roda's Kitchen, and the generous contributions of our community, $16,000 was raised, setting a new benchmark for support.

The Easter Cookies fundraiser saw an overwhelming response, with an astounding 800 boxes sold, equating to a remarkable 800 meals on wheels supplied to the county of Northumberland. This achievement marks a significant milestone, demonstrating the power of collective action in addressing the needs of our community's most vulnerable members.

"We are overwhelmed by the outpouring of support from our community, donors, volunteers, and the incredible team at Roda's Kitchen," expressed Trish Baird, CEO of CCN. "Together, we have ensured that 800 individual meals will reach residents in our community who have access to nutritious meals through Meals on Wheels."

Meals on Wheels remains a cornerstone program offered by CCN, providing vital sustenance to seniors and adults with disabilities who are unable to prepare meals for themselves. The funds raised through the Easter Cookies fundraiser will directly contribute to the continuity and expansion of this critical service, ensuring that no one in our community goes hungry.

About CCN’s Meals on Wheels Program

Community Care Northumberland’s Meals on Wheels program delivers healthy and affordable meal options to clients throughout Northumberland County. People can choose to receive hot meals, which are prepared locally and delivered several times per week at the noon hour, and/or frozen meals, which are easy to prepare and cater to several diet types. All meals are delivered by CCN volunteers or staff members right to the client’s door. In 2022-2023, our Meals on Wheels program delivered 40,421 meals to 636 clients in the county.

Community Care Easter Cookies for Meals on Wheels launches Friday, March 1st

FOR IMMEDIATE RELEASE: Northumberland County, ON – Feb. 26, 2024 - In celebration of Nutrition Month and March for Meals and just in time for the Easter holiday, Community Care Northumberland (CCN) is excited to announce the release of Easter cookie boxes in support of its Meals on Wheels program.

Each box contains 5 individually wrapped, beautifully decorated Easter cookies from Roda’s Kitchen, priced at $20.00 to support CCN’s Meals on Wheels program. This initiative provides essential frozen and hot meal deliveries to clients throughout Northumberland County. Of each $20.00 cookie box sold, $8.00 will directly support the Meals on Wheels program.

March marks National Nutrition Month and the annual March for Meals Month, pivotal moments for CCN to advocate for the significance of its Meals on Wheels program within the region. CCN dutifully delivers approximately 3,000 meals monthly to residents in Northumberland.

In addition to offering these delightful cookies, local Mayors, volunteers, and other community members will join CCN for the 27th anniversary of March for Meals this year, demonstrating their support by delivering meals as 'Community Champions' to our clients.

“The cookie fundraiser and March for Meals initiative are essential components in meeting the nutritional requirements of our clients and community, simultaneously amplifying awareness about this invaluable program,” said Trish Baird, Chief Executive Officer of Community Care Northumberland.

Community Care Northumberland’s Seventh Annual Adopt a Grandparent Program Begins

FOR IMMEDIATE RELEASE: Northumberland County, ON – November 1, 2023 – For the seventh consecutive year, Community Care Northumberland (CCN) will support seniors throughout Northumberland County this holiday season through the organization’s Adopt a Grandparent program.

This program spreads joy by surprising seniors who may be experiencing isolation or loneliness with meaningful gifts during the holiday season.

Trish Baird, Chief Executive Officer for Community Care Northumberland, comments, “Adopt a Grandparent has shown us what coming together as a community can be all about and how nice it is to give back: in the true holiday spirit, our community unites each year, bringing some happiness, joy and connections to those who need it most.”

Hear what our Adopted Grandparents have to say:

This year, members of the community who are interested in adopting a grandparent can do so by calling Community Care Northumberland at 1-866-514-5774 or by emailing wellness@commcare.ca to be matched with a senior. Purchased gifts may be dropped off at a local office to be delivered to our “Grandparents” by our volunteers the week before Christmas. Community Care Northumberland asks that all unwrapped gifts (gift bags or baskets are ok) be dropped off at a local office on or before December 8th. Individuals may also “Adopt a Grandparent” by donating to this program and allowing Community Care Northumberland staff to complete the shopping.

We kindly ask participants to refrain from including homemade baked goods. We also ask that you include washing instructions for any handmade items, such as scarves or mittens. Other gift suggestions include gift cards for local stores or gift certificates for Community Care Northumberland programs, such as Meals on Wheels or Transportation.

Community Care Northumberland Celebrates its 35th Anniversary

FOR IMMEDIATE RELEASE: Northumberland County, ON – July 20, 2023

The non-profit organization celebrates its 35th anniversary this August 22nd, 2023

For over three and a half decades Community Care Northumberland (CCN) has been providing essential services and programs to Northumberland County residents. With the amalgamation of five local agencies (Brighton, Campbellford, Cobourg, Colborne, and Port Hope), CCN was formed to provide community support services to give people the opportunity to live well, at home with the assistance of screened and trained volunteers.

In 2010 another integration was successfully facilitated with two local organizations (Hospice Northumberland Lakeshore and Palliative Care Campbellford) and CCN began offering hospice services, to complement community support services being provided. In 2017 the Palliative Care Community Team was established with clinical navigation, supportive care and grief and bereavement services added to visiting hospice services. Later that year, approval was given to build the Northumberland Hospice Care Centre, led by CCN. By September of 2020 the first residents were admitted to Ed’s House for end-of-life palliative care. Ed’s House Northumberland Hospice Care Centre currently has six resident suites and has four ‘roughed-in’ suites for future expansion.

A new program was also formed in 2020, when CCN partnered with the Ontario Health Teams of Northumberland (OHT-N) and launched WrapAround for Older Adults. This program matches trained peer support facilitators with older adults, supporting those experiencing persistent complex needs and conditions and their caregivers.

In the past year, CCN has completed 32,000 essential drives for clients in our community, provided 15,398 social and safety calls, and delivered 40,421 nutritious meals. Hospice volunteers and staff supported 531 clients in the community and over 148 residents and their families at Ed’s House. CCN’s Brokered Worker program assisted almost 500 home help and maintenance clients. The WrapAround program has had six successful participants transition out of the program and currently has six active participants with ten more ready to be matched with volunteer facilitators.

With an aging population in Northumberland County and an increased desire for individuals to stay in their homes longer, CCN’s service needs are increasing. In the past year (April 2022-March 2023), Community Care Northumberland has received a total of 362 new referrals for programs and services with 72% of these requests from individuals aged 65 and older.

To support these needs, CCN is calling for individuals who have time to give to their community. For as little as one (1) hour per week, you can make an impact in your community.

The Easter Cookies fundraiser saw an overwhelming response, with an astounding 800 boxes sold, equating to a remarkable 800 meals on wheels supplied to the county of Northumberland. This achievement marks a significant milestone, demonstrating the power of collective action in addressing the needs of our community's most vulnerable members.

Community Care Easter Cookies for Meals on Wheels launches Friday, March 1st

FOR IMMEDIATE RELEASE: Northumberland County, ON – Feb. 26, 2024 - In celebration of Nutrition Month and March for Meals and just in time for the Easter holiday, Community Care Northumberland (CCN) is excited to announce the release of Easter cookie boxes in support of its Meals on Wheels program.

Each box contains 5 individually wrapped, beautifully decorated Easter cookies from Roda’s Kitchen, priced at $20.00 to support CCN’s Meals on Wheels program. This initiative provides essential frozen and hot meal deliveries to clients throughout Northumberland County. Of each $20.00 cookie box sold, $8.00 will directly support the Meals on Wheels program.

March marks National Nutrition Month and the annual March for Meals Month, pivotal moments for CCN to advocate for the significance of its Meals on Wheels program within the region. CCN dutifully delivers approximately 3,000 meals monthly to residents in Northumberland.

In addition to offering these delightful cookies, local Mayors, volunteers, and other community members will join CCN for the 27th anniversary of March for Meals this year, demonstrating their support by delivering meals as 'Community Champions' to our clients.

Community Care Northumberland’s Seventh Annual Adopt a Grandparent Program Begins

FOR IMMEDIATE RELEASE: Northumberland County, ON – November 1, 2023 – For the seventh consecutive year, Community Care Northumberland (CCN) will support seniors throughout Northumberland County this holiday season through the organization’s Adopt a Grandparent program.

This program spreads joy by surprising seniors who may be experiencing isolation or loneliness with meaningful gifts during the holiday season.

Trish Baird, Chief Executive Officer for Community Care Northumberland, comments, “Adopt a Grandparent has shown us what coming together as a community can be all about and how nice it is to give back: in the true holiday spirit, our community unites each year, bringing some happiness, joy and connections to those who need it most.”

Hear what our Adopted Grandparents have to say:

This year, members of the community who are interested in adopting a grandparent can do so by calling Community Care Northumberland at 1-866-514-5774 or by emailing wellness@commcare.ca to be matched with a senior. Purchased gifts may be dropped off at a local office to be delivered to our “Grandparents” by our volunteers the week before Christmas. Community Care Northumberland asks that all unwrapped gifts (gift bags or baskets are ok) be dropped off at a local office on or before December 8th. Individuals may also “Adopt a Grandparent” by donating to this program and allowing Community Care Northumberland staff to complete the shopping.

We kindly ask participants to refrain from including homemade baked goods. We also ask that you include washing instructions for any handmade items, such as scarves or mittens. Other gift suggestions include gift cards for local stores or gift certificates for Community Care Northumberland programs, such as Meals on Wheels or Transportation.

Community Care Northumberland Celebrates its 35th Anniversary

FOR IMMEDIATE RELEASE: Northumberland County, ON – July 20, 2023

The non-profit organization celebrates its 35th anniversary this August 22nd, 2023

For over three and a half decades Community Care Northumberland (CCN) has been providing essential services and programs to Northumberland County residents. With the amalgamation of five local agencies (Brighton, Campbellford, Cobourg, Colborne, and Port Hope), CCN was formed to provide community support services to give people the opportunity to live well, at home with the assistance of screened and trained volunteers.

In 2010 another integration was successfully facilitated with two local organizations (Hospice Northumberland Lakeshore and Palliative Care Campbellford) and CCN began offering hospice services, to complement community support services being provided. In 2017 the Palliative Care Community Team was established with clinical navigation, supportive care and grief and bereavement services added to visiting hospice services. Later that year, approval was given to build the Northumberland Hospice Care Centre, led by CCN. By September of 2020 the first residents were admitted to Ed’s House for end-of-life palliative care. Ed’s House Northumberland Hospice Care Centre currently has six resident suites and has four ‘roughed-in’ suites for future expansion.

A new program was also formed in 2020, when CCN partnered with the Ontario Health Teams of Northumberland (OHT-N) and launched WrapAround for Older Adults. This program matches trained peer support facilitators with older adults, supporting those experiencing persistent complex needs and conditions and their caregivers.

In the past year, CCN has completed 32,000 essential drives for clients in our community, provided 15,398 social and safety calls, and delivered 40,421 nutritious meals. Hospice volunteers and staff supported 531 clients in the community and over 148 residents and their families at Ed’s House. CCN’s Brokered Worker program assisted almost 500 home help and maintenance clients. The WrapAround program has had six successful participants transition out of the program and currently has six active participants with ten more ready to be matched with volunteer facilitators.

With an aging population in Northumberland County and an increased desire for individuals to stay in their homes longer, CCN’s service needs are increasing. In the past year (April 2022-March 2023), Community Care Northumberland has received a total of 362 new referrals for programs and services with 72% of these requests from individuals aged 65 and older.

To support these needs, CCN is calling for individuals who have time to give to their community. For as little as one (1) hour per week, you can make an impact in your community.

Community Care Northumberland’s Seventh Annual Adopt a Grandparent Program Begins

FOR IMMEDIATE RELEASE: Northumberland County, ON – November 1, 2023 – For the seventh consecutive year, Community Care Northumberland (CCN) will support seniors throughout Northumberland County this holiday season through the organization’s Adopt a Grandparent program.

This program spreads joy by surprising seniors who may be experiencing isolation or loneliness with meaningful gifts during the holiday season.

Trish Baird, Chief Executive Officer for Community Care Northumberland, comments, “Adopt a Grandparent has shown us what coming together as a community can be all about and how nice it is to give back: in the true holiday spirit, our community unites each year, bringing some happiness, joy and connections to those who need it most.”

This year, members of the community who are interested in adopting a grandparent can do so by calling Community Care Northumberland at 1-866-514-5774 or by emailing wellness@commcare.ca to be matched with a senior. Purchased gifts may be dropped off at a local office to be delivered to our “Grandparents” by our volunteers the week before Christmas. Community Care Northumberland asks that all unwrapped gifts (gift bags or baskets are ok) be dropped off at a local office on or before December 8th. Individuals may also “Adopt a Grandparent” by donating to this program and allowing Community Care Northumberland staff to complete the shopping.

We kindly ask participants to refrain from including homemade baked goods. We also ask that you include washing instructions for any handmade items, such as scarves or mittens. Other gift suggestions include gift cards for local stores or gift certificates for Community Care Northumberland programs, such as Meals on Wheels or Transportation.

Community Care Northumberland Celebrates its 35th Anniversary

The non-profit organization celebrates its 35th anniversary this August 22nd, 2023

For over three and a half decades Community Care Northumberland (CCN) has been providing essential services and programs to Northumberland County residents. With the amalgamation of five local agencies (Brighton, Campbellford, Cobourg, Colborne, and Port Hope), CCN was formed to provide community support services to give people the opportunity to live well, at home with the assistance of screened and trained volunteers.

In 2010 another integration was successfully facilitated with two local organizations (Hospice Northumberland Lakeshore and Palliative Care Campbellford) and CCN began offering hospice services, to complement community support services being provided. In 2017 the Palliative Care Community Team was established with clinical navigation, supportive care and grief and bereavement services added to visiting hospice services. Later that year, approval was given to build the Northumberland Hospice Care Centre, led by CCN. By September of 2020 the first residents were admitted to Ed’s House for end-of-life palliative care. Ed’s House Northumberland Hospice Care Centre currently has six resident suites and has four ‘roughed-in’ suites for future expansion.

A new program was also formed in 2020, when CCN partnered with the Ontario Health Teams of Northumberland (OHT-N) and launched WrapAround for Older Adults. This program matches trained peer support facilitators with older adults, supporting those experiencing persistent complex needs and conditions and their caregivers.

In the past year, CCN has completed 32,000 essential drives for clients in our community, provided 15,398 social and safety calls, and delivered 40,421 nutritious meals. Hospice volunteers and staff supported 531 clients in the community and over 148 residents and their families at Ed’s House. CCN’s Brokered Worker program assisted almost 500 home help and maintenance clients. The WrapAround program has had six successful participants transition out of the program and currently has six active participants with ten more ready to be matched with volunteer facilitators.

With an aging population in Northumberland County and an increased desire for individuals to stay in their homes longer, CCN’s service needs are increasing. In the past year (April 2022-March 2023), Community Care Northumberland has received a total of 362 new referrals for programs and services with 72% of these requests from individuals aged 65 and older.

To support these needs, CCN is calling for individuals who have time to give to their community. For as little as one (1) hour per week, you can make an impact in your community.

CCN’s ‘Caring Fore Our Community’ Golf Tournament Raises $22,461 to Support Vital Programs and Services

Northumberland County, July 4, 2023 - Community Care Northumberland (CCN) is thrilled to announce the successful outcome of its annual 'Caring Fore Our Community' Golf Tournament, held on June 22, 2023. With the support of generous community sponsors, donors and golfers, the event raised an impressive $22,461, ensuring the continued affordability and accessibility of CCN's programs and services for clients throughout Northumberland County.

CCN is committed to providing essential services such as nutrition, transportation, wellness, and in-home assistance to seniors, adults with disabilities, individuals recovering from illness or injury, as well as hospice clients and their loved ones and caregivers.
