WrapAround Facilitator

Contract – 3 years

Reporting to the Community Care Northumberland Volunteer Peer Support Coordinator, the WrapAround Facilitator is responsible for delivering a high-fidelity WrapAround process to support vulnerable older adults and their caregivers. This role coordinates and facilitates a team-based approach to “wrap” participants in essential community support. Through the development and monitoring of individualized plans, the Facilitator empowers participants to build on their strengths and remain independent in their homes. All work is conducted to embrace the mission, vision and core values of Community Care Northumberland.

Primary Duties and Responsibilities:

  • Lead the full WrapAround process, including Engagement, planning, implementation and transition.
  • Facilitate team meetings.
  • Develop & implement individualized, strength based plans across key life domains (e.g. housing, health, financial and social)
  • Work closely with program volunteers who provide supportive roles to participants providing guidance, communication and ongoing collaboration.
  • Build & maintain strong relationships with community partners & referral agencies.
  • Maintain accurate, timely and confidential client records in accordance with agency policies
  • Monitor client progress and support appropriate referrals to meet evolving needs.
  • Participate in the WrapAround community of practice for ongoing learning and peer support. 

Qualifications:

  • Education – Successful completion of a post-secondary degree or diploma in Social Work, Social Services, or a related field.
  • Minimum 3 years’ direct or related experience with client centered planning.
  • Demonstrated knowledge of systems theory and the ability to navigate community resources/services.
  • Strong interpersonal, conflict resolution, and facilitation skills to build trust with diverse participants.
  • Excellent written and oral communication skills, including proficiency in Microsoft Office Suite.
  • Proven ability to work independently and as a cooperative member of a collaborative team.
  • Valid Ontario Driver’s license (Full G Class) and use of a reliable vehicle for travel throughout Northumberland County.
  • Satisfactory Vulnerable Sector Police Check
  • Completion of the intensive 4-day WrapAround training program (or a commitment to complete it).
  • First Aid/CPR Training Certificate is an asset
  • Health and safety policy knowledge
  • Strong organizational skills and focus to manage competing priorities
  • Experience working with volunteers is an asset

Starting salary range – $21.94 to $23.27
