Personal Support Worker

Reporting to the Clinical Manager, the Personal Support Worker  (PSW)  will be responsible for providing professional, evidence-based, client-centred care for clients admitted to Ed’s House Northumberland Hospice Care Centre, in accordance with Community Care Northumberland and Hospice Palliative Care Ontario standards.

The PSW will work in collaboration with the interdisciplinary team.  They will provide care and be part of a care delivery team with, but not limited to the Clinical Manager, Medical Director, Physicians, Registered Nurse, Registered Practical Nurse, Supportive Care and Grief and Bereavement Counsellors, Food Services along with other support staff and volunteers within Ed’s House. 

Primary Duties and Responsibilities:

  • Providing services guided by a model of care that is holistic, collaborative in nature and aligns with current evidence-based practice, as supported by HPCO standards.
  • Care to residents living at Ed’s House.*
  • Care of the hospice environment.*
  • Monitor, document and report changes in resident needs and concerns to appropriate nursing staff.
  • Contributes to the ongoing review and implementation of the resident care plan.
  • Charts in EMR personal care provided.
  • Recognizes the importance of maintaining a home-like environment, contributing to the daily operation of the hospice in a team like atmosphere.
  • When a resident is no longer able to contribute to their care, continuing to provide care in a way that maintains their dignity, well-being, and self-image.
  • Recognize signs of active dying and communicates with appropriate staff regarding the residents changing needs.
  • Participates in mandatory education/training in-services for skill development and updates on new and/or relevant information on best practices.
  • Actively participates on internal and external committees and always represents Community Care Northumberland/Ed’s House in a professional manner.
  • Commits to meeting the privacy obligations and requirements of the agency as a custodian of Personal Health Information.
  • Actively participates and complies with health and safety policies, procedures and legislation.
  • Promotes the philosophy and goals of Community Care Northumberland.
  • Contributes to team effectiveness and agency success.

Qualifications:

  • Must hold a certificate as a Personal Support Worker from a curriculum approved by the Ministry of Training, Colleges and Universities or the Ministry of Education.
  • Registered in good standing with the Personal Support Worker Registry of Ontario
  • Valid CPR/First Aid Certification
  • Completed palliative education (PACE, Fundamentals of Palliative Care, LEAP, or equivalent educational courses).  Course completion of PACE is an on-the-job requirement.
  • Demonstrated experience in working with an inter-disciplinary, multi-dimensional care team.
  • Effective time management skills, ability to prioritize tasks and follow direction.
  • Demonstrated experience providing hospice palliative care in community, hospital, or appropriate long-term care setting.
  • Evidence of a demonstrated client centered approach to care and ability to work with families, being able to adapt to communication challenges.
  • Understanding of therapeutic relationships and how to maintain professional boundaries.
  • Experience collaborating with volunteers in a team environment.
  • Thorough understanding of Hospice Palliative Care Ontario standards.
  • Demonstrated ability to display a sense of calm while working under pressure.
  • Excellent conflict resolution skills.
  • Strong computer skills (Word, Excel, Outlook, databases, and effective internet research) and experience with electronic medical records.
  • Ability to work a minimum 8-hour work schedule, be in good health and physically fit.
