Community Care Northumberland Office Closures Due to COVID-19

Community Care Northumberland offices are closed to the public in an effort to support social distancing and to protect our staff, volunteers and clients.

Please note that we are still operational and can be reached by phone or email. Contact information can be found in the ‘Contact’ tab at the top of this page.

At this time payments can be mailed or credit card payments can be taken over the phone.

Home at Last

Home at Last

A one-time settlement home from the hospital that can include transportation, grocery shopping, medication pick up, preparing a small meal and providing personal care and/or homemaking.  Qualifying clients may receive enhanced supports to make the transition from hospital to home a success.

Meals on Wheels

Meals on Wheels

Hot Meals on Wheels program suspended. We will revert back to offering frozen meals only with deliveries being completed by CCN staff.

Community diners are still suspended.

Transportation

Transportation

Specialized transportation continues to operate for essential transportation only.

Safety measures, including the mandatory use of masks and vehicle cleaning between clients, are in place.

Friendly Visiting

Friendly Visiting

A visit or telephone call from a caring volunteer on a regular basis to share in activties and campanionship.  * Please note at this time all our visits are virtual via telephone.

A monthly program that offers a tasty, nutritious, affordable meal enjoyed with others.  It's a time for fun, entertainment and learning that keeps you connected with your community.

Hospice Palliative Care

Hospice Palliative Care

Support is provided to anyone with a life limiting illness.  Family and friends can access support from trained volunteers, a nurse, and counsellor.  Individual grief support and grief groups are available.

Wellness Programs

Wellness Programs

Overall health and wellness of clients is promoted through a variety of activities and programs such as fitness, blood pressure clinics, choral groups, support groups and more.

Through COVID, online virtual classes are available.

Telephone Security Checks

Telephone Security Checks

A short telephone call at a prearranged time will provide reassurance, a sense of security or send assistance when needed.

Support for Caregivers

Support for Caregivers

Assistance in meeting the individual needs of caregivers with their on-going responsibilities associated with caring for a loved one, a friend or a neighbour.

Home Help Maintenance

Home Help Maintenance

Home Help provides assistance with household activities such as light housekeeping, shopping and meal preparation.  Home Maintenance provides assistance with heavier chores such as minor home repairs, snow shovelling and yeard maintenance.  These services are provided by brokered workers.

The Goal is comfort, not cure where people and their families will be supported with care, compassion and expertise every step of the way.  Ed's house will be a two storey facility located in Hamilton Township at the cross roads of Ontario Street and Oliver's Lane.
