Caregiver Support

Our Hospice Services are able to provide on-going support, comfort and care to the caregivers, friends and family of persons with a life limiting diagnosis. These supports are coordinated by Hospice Services staff and can include:

Education

Palliative care aims to address physical, psychological, emotional, spiritual, and clinical needs of a person with a life limiting illness. Our Hospice Services aims to educate patients, families, caregivers, and health service providers about the benefits of a holistic approach to care.

For Patients, Families and Caregivers:

Our Hospice Services is able to provide education to patients, families, and caregivers on how the palliative care approach can support them at home.  This may include suggestions for home management, self-care strategies, what to expect during disease progression, options for the types of in home services they may qualify for, and what to expect when having a death at home

For Health Service Providers:

Hospice Services is able to provide education sessions for interested health service providers on how the palliative care approach can support their practice, establishing a substitute decision maker, and how to start discussions for advanced care planning.

Grief and Bereavement Support

Our Hospice Services are able to provide on-going support, comfort and care to the caregivers, friends and family of persons who have passed away. These supports are coordinated by our Hospice Services staff and can include:

  • support for friends and family, including grief support and respite
  • anticipatory grief support
  • compassionate presence
  • vigil support
  • telephone support
  • information and referral
  • access to library resources
  • child and teen support
  • Counselling
  • Meals on Wheels
  • Transportation

Health System Navigation

Deciding what kinds of help you or a loved one need during an illness can be challenging.

There may be medical, emotional, spiritual, physical, and psychological needs to be addressed.

Knowing who to speak to, which agency is best suited to meet your needs, and whether you qualify for service are things Hospice Services’ Clinical Navigator can help with.

Our Hospice Clinical Navigator is a nurse who helps identify services that will support the goals of a person’s care. Their role is to act as a constant point of contact for patients and caregivers throughout an illness. They are available to answer questions, suggest services and supports that may ease the burden of living with an illness and help organize care to reflect your or your loved one’s wishes.

For Health Service Providers

Hospice Services are able to provide on-going support, comfort and care to the caregivers, friends and family of persons with a life limiting diagnosis. These supports are coordinated by Hospice Services staff and can include:

Navigation Support:

The Hospice Clinical Navigator (R.N.) is a LHIN funded position who works to holistically assess a patient’s needs continuously from the point of diagnosis until passing. When a need is identified, the Navigator works collaboratively to develop and implement a plan which best reflects the patient’s goals of care, making referrals to community partners where appropriate.

Bereavement Counselling:

Hospice Services employs a Supportive Care Counsellor (RSW) to provide psychological, emotional, social and spiritual counselling to family members and caregivers experiencing bereavement outside the scope of Hospice Services’ visiting volunteers or bereavement support group.
