The Path Project:

Bereavement Ontario Network - is a diverse group of organizations and individuals throughout the province that work in the field of grief, bereavement, and mourning as professionals and volunteers.

Hospice Palliative Care Ontario - provides information on community-based, volunteer organizations providing end-of-life care for individuals living with a life-threatening illness, and those who care for them.

NEARN - Northumberland Elder Abuse Response Network 

Northumberland Family Health Team - patient-centred primary care organizations that offer patients access to a number of primary health care services from a variety of team members, beyond a family doctor. The Northumberland Family Health Team (NFHT) is a doctor-led Family Health Team currently made up of 21 local family doctors who work together with other health care professionals to co-ordinate the highest possible quality of care for their enrolled patients.

Northumberland Hills Hospital - hospital located in Cobourg, Ontario.

The PATHway to Aging Well has been designed to bring current and valid information, resources and planning tools together for the Northumberland Community. The information contained in this website was built by a team of seniors, caregivers and providers, with the support of other community partners.  

Port Hope Northumberland Coummunity Health Centre - a community centred primary health care facility, and a member of The Association of Ontario Health Centres (AOHC), the advocacy organization for non-profit, community governed, multidisciplinary primary health care models. 

South East Regional Elder Abuse Prevention Planning (REAP).  This website guides service providers or people with questions who may be experiencing elder abuse to services that can help who operate within the South East Region.

Safe Communities Northumberland County - Mission:  To make Northumberland County the safest and healthiest place to live, learn, work and play. Vision:  To support the establishment and implementation of a coordinated approach to addressing safety issues in Northumberland County, including Falls Prevention, On/Off Road Safety and Self-Harm/Interpersonal Violence.

Trent Hills Family Health Team - (THFHT) works out of five locations in Campbellford, Hastings, Warkworth, Colborne and Havelock. Our main site (head office) is located in the Campbellford Memorial Health Centre at 119 Isabella Street in Campbellford.  The THFHT delivers primary health care, programs and services to approximately 14,000 patients in the Municipality of Trent Hills and adjacent areas.

The Bridge Hospice - A tranquil, personal setting where people near the end of life and their loved ones can share precious time when it matters most.  An inter-professional healthcare team provides around the clock bedside care while volunteers offer compassion, kindness and the comforts of home.

VON Canada - Adult Day Program - Will serve older adults in the community who need special support to remain living at home.
