CentralEasthealthline.ca is an innovative website that puts accurate and up-to-date information about health services at the fingertips of consumers and health care providers across the Central East region of Ontario.

Health Links  - The Central East LHIN has brought together health care providers across different sectors to form several Health Links working on improving care coordination. Each Health Link is a local health care network consisting of patients, caregivers, health care providers and community support agencies who are committed to working better together to improve the health outcomes for patients with complex health care needs.

Central East LHIN

Mission of the Central East LHIN: To lead the advancement of an integrated sustainable health care system that ensures better health, better care and better value.

The Central East LHIN is one of the fastest growing geographic regions in the Province and home to over 11% of Ontario’s population.  Based on the 2006 Census, the population has grown by 6.3 percent, making the LHIN 1.4 million people strong. 

The Central East LHIN is a mix of urban and rural geography and is the sixth-largest LHIN in land area in Ontario (16,673 km2).  In densely populated urban cities, suburban towns, rural farm communities, cottage country villages and remote settlements, the Central East LHIN stretches from Victoria Park to Algonquin Park!  The neighbourhoods in our planning zones boast a rich diversity of community values, ethnicity, language and socio-demographic characteristics.

Central East Local Health Integration Network 


Community Care Northumberland receives 50% of its base funding from the Local Health Integration Networks.  The Central East LHIN covers the following municipalities within Community Care Northumberland's service area: 

  • Alnwick - Haldimand Township
  • Cramahe Township
  • Hamilton Township
  • Municipality of Port Hope
  • Municipality of Trent Hills
  • Town of Cobourg

Together with the Central East LHIN, Community Care Northumberland delivers non – medical home support services to :

  • Seniors,
  • Adults with disabilities, or
  • All persons (or their caregivers) who has been diagnosed with a terminal illness; 

so that they can remain healthy and independent in their own homes.
